About Us

Illinois MENTOR is a trusted provider of home and community-based human services. Our personalized services, including Host Home, Supported Living, and In-Home Supports, are designed to help adults of all abilities thrive in the communities they call home.

Many of the individuals we support come to us from more restrictive settings, including institutions, hospitals and detention centers.  We offer smaller, more nurturing residential programs and family home settings designed to help people build relationships.  Working collaboratively, our individualized care and treatment plans are developed in consultation with clinical professionals, family members and Host Home providers as well as the individuals we support.

Quality is at the heart of everything we do.  Our statewide Quality Improvement team establishes clear goals, benchmarks and outcomes for every service we offer and every employee who works for us.  In addition, our team works to ensure we meet or exceed our quality of care standards as well as find new and better ways to safely and compassionately serve the people who depend on us.
